Family First Support Services is a community-based organization dedicated to helping individuals and families thrive. We simplify the process of finding and qualifying for community and government-funded programs by providing personalized guidance and support. We remove red tape with compassionate, personalized guidance so you get what you need, faster.

Our staff coordinate appointments, referrals, and follow ups—connecting you to primary care, specialists, behavioral health clinics, and community providers.
We help you understand eligibility and enroll in Medicaid, Medicare, or Marketplace plans.
We guide SNAP applications, connect you to local food pantries, and help you find housing resources.
We help you prepare documentation, understand eligibility, and navigate appeals while connecting you to legal aid when needed.
Résumé help, job searches, community training programs, and referrals to workforce development partners.
We streamline referrals for developmental evaluations, therapy services, and school based supports.
Connections to physical, occupational, and speech therapy providers.
We help you assess needs and enroll in adult day programs, CDPAP/home care options, and caregiver support resources.
Family First Support Services, in partnership with the Hudson Valley Care Coalition, and the NYS Social Care Network program, partners with local organizations to create a strong social care network.
The Social Care Network (SCN) ensures individuals and families receive holistic support beyond healthcare. We address social determinants of health by connecting you to essential community resources—because well-being is more than medical care.
We identify social challenges such as food insecurity, housing instability, and transportation barriers during intake.
Our team guides you through eligibility requirements and application processes for community programs.
We work with multiple agencies to ensure services are aligned and delivered without gaps.
We connect you directly to local providers for: Food Assistance, Housing Support & Transportation Services.
